Can Transarterial Chemoembolization of Hepatocellular Carcinoma Result in Transformation to Combined Hepatocellular-Cholangiocarcinoma with Stem Cell Features? A Case Study

چکیده
Transarterial chemoembolization (TACE) is currently one of the favored treatment modalities for noncurative hepatocellular carcinoma (HCC) and can be used to shrink tumor size in order to make a patient eligible for transplantation. Furthermore, with the advent of effective antiviral drugs for hepatitis B virus (HBV), concomitant antiviral therapy with local tumor ablation including TACE for HBV-associated HCC has been successful for long term survival. Over the last decade, however, concern has been raised about a phenomenon whereby a subset of TACE-treated HCCs becomes more aggressive after TACE treatment. One current hypothesis is that TACE eliminates only the hepatocellular cells and that hepatic progenitor cells that have the potential for developing to cholangiocarcinoma are then selected for and induced to proliferate postTACE with possible dual differentiation along hepatocellular and biliary lines. We present a case of a patient with HCC who underwent TACE and subsequently experienced tumor regrowth as biopsy-proven combined hepatocellular-cholangiocarcinoma. Furthermore, we provide immunohistochemical evidence of hepatic progenitor cells in the post-TACE tumor biopsy, possibly accounting for its aggressive course. [N A J Med Sci. 2017;10(4):181-186. Sarcomatoid combined hepatocellular-cholangiocarcinoma: a case report and review of literature.
Sarcomatoid combined hepatocellular-cholangiocarcinoma is an extremely rare primary hepatic malignancy and only a few cases have been reported. Herein, we report a case of combined hepatocellular-cholangiocarcinoma with sarcomatoid changes in a 52-year-old man, who had a history of liver cirrhosis and transarterial chemoembolization. The resected liver revealed a mass of 4.5×3.5 cm. Microscopic...
متن کاملClinical features, histology, and histogenesis of combined hepatocellular-cholangiocarcinoma
Combined hepatocellular-cholangiocarcinoma (CHC) is a rare tumor with poor prognosis, with incidence ranging from 1.0%-4.7% of all primary hepatic tumors. This entity will be soon renamed as hepato-cholangiocarcinoma. The known risk factors for hepatocellular carcinoma (HCC) have been implicated for CHC including viral hepatitis and cirrhosis. It is difficult to diagnose this tumor pre-operativ...
متن کاملClinicopathological characteristics and prognostic factors in combined hepatocellular carcinoma and cholangiocarcinoma
BACKGROUNDS/AIMS Combined hepatocellular carcinoma and cholangiocarcinoma (cHCC-CC) is an uncommon subtype of primary liver cancer that has rarely been reported in detail. This study was performed in order to evaluate the clinicopathological characteristics and prognostic factors of cHCC-CC in single center. METHODS The clinicopathological features of patients diagnosed and operated with cHCC...
متن کاملCombined hepatocellular-cholangiocarcinoma with stem cell features, cholangiolocellular subtype after inferior vena cava stent placement for a patient with Budd–Chiari syndrome
We report a case of combined hepatocellular-cholangiocarcinoma with stem cell features, cholangiolocellular subtype arising about 15 years after placement of an inferior vena cava stent for primary Budd-Chiari syndrome. Pre-surgical differentiation of the tumor from hepatocellular carcinoma was difficult because of elevated levels of alpha-fetoprotein and hypervascularity in the arterial phase....
متن کاملTransarterial Ethanol Ablation Combined with Transarterial Chemoembolization for Hepatocellular Carcinoma with Portal Vein Tumor Thrombus
BACKGROUND The prognosis for hepatocellular carcinoma (HCC) with portal vein tumor thrombosis (PVTT) is extremely poor. OBJECTIVES This study aimed to evaluate the safety and effectiveness of transarterial ethanol ablation (TEA) for the treatment of HCC with PVTT. METHODS Patients were treated with TEA for PVTT under cone-beam computed tomography and traditional transarterial chemoembolizat...
